MSWD Services

 

  • Filled- up request for Reconnection. Authorization letter from the registered connection owner allowing the client to reconnect his/her connection (if the owner is unable to file in the office).
    • Reconnection Fee – ₱ 150.00
  • Submit filled-up request for reconnection form.
  • Pay the reconnection fee and issuance of Offical Receipt. (OR).
  • Personnel proceed to the water meter of the consumer and reconnects water services

 

 

  • Filled- up request for Relocation. Authorization letter from the registered connection owner allowing the client to relocate his/her connection (if the owner is unable to file in the office).
    • Relocation Fee – ₱ 200.00
  • Proceed to the Customer Service Assistance Desk for evaluation of  the clustering where water meter is to be transferred
  • If passed on the evaluation, pay the relocation fee to the cashier at the cashiering section and ask for Official Receipt (OR)
  • Ask for the schedule of relocation.

 

 

  • Authorization letter from the registered connection owner allowing the client to change the account name to his/her name.
  •  Proceed to the Customer Service Assistance Desk to Submit the required document

 

 

  • Bring Xerox Copy of S.C. ID Filled-up application for availment of Senior Citizen Discount
  • Proceed to the Customer Service Assistance Desk.
  • Submit all the requirements.
  • Attend for orientation of policy in availment of SC Discount.
